Background & objectives:Helicobacter pylori (H. pylori) is a Gram‑negative bacterium associated with chronic gastritis, peptic ulcer disease, and gastric cancer. This study assessed the prevalence of H. pylori, its associated risk factors, and the frequency of gastric adenocarcinoma among patients attending an endoscopy unit in south Tehran. Methods: In this cross-sectional study, 500 patients who were referred to the endoscopy department of Mofarrah Hospital in south Tehran were enrolled. Helicobacter pylori (H. pylori) infection was screened using a urea breath test and was confirmed through gastric biopsy, rapid urease test, and histopathological examination. The data collected were analyzed using STATA statistical software. The relationship between H. pylori infection and various demographic and clinical factors was examined, with a significance level set at p< 0.05. Results: The overall prevalence of H. pylori was found to be 34% (170 out of 500 participants). The infection was significantly associated with tobacco use and lower education levels (p<0.05). However, no significant associations were identified between H. pylori prevalence and factors such as obesity, age, sex, or gastric adenocarcinoma. Conclusion: One-third of the patients undergoing endoscopy at this center tested positive for H. pylori. Higher infection rates were associated with tobacco use and lower education levels. These findings can help shape local public health strategies aimed at targeting specific risk factors, improving screening processes, and guiding timely eradication efforts to reduce morbidity related to H. pylori.
